How to Purchase Tickets
Cheer on Broncos and enjoy regular home games at a discounted rate with season passes. Tickets go on sale starting August 6, 2024.

Final Forms Paperwork
Student-athletes use Final Forms to complete most paperwork. Register here.
On Final Forms, Click “New Account” under the parent icon. After you have confirmed your registration, add the following for your student-athlete(s):
Basic Medical History & Health Information
Insurance Company & Policy Number
Doctor & Dentist Contact Information
Hospital Preference
Email Address for you and your student
After your initial set up, you will not have to complete this information again. You will only need to update the information if there are changes.

Physical Forms
Students must have an OHSAA physical form, completed and signed by a doctor, on file in the school office or uploaded onto Final Forms prior to tryouts or practice
You may come to open gyms/mats without a completed physical. If we don’t have a copy of your completed physical, you will not be able to tryout or practice with any team
Success Beyond the Classroom Scholarships
Does your middle school child plan to play sports? There is scholarship money made available through the Success Beyond the Classroom Foundation (SBC) to assist in covering the Pay-to-Participate cost. SBC is a non-profit organization that raises funds to provide scholarships to qualified students in the district who wish to participate in athletics and/or marching band.
SBC members believe that extra-curricular activities help students learn and practice the skills needed for success in all areas of life. To be considered for an SBC scholarship, students must submit and fully complete an application. Click here for the application.

Affiliation
Beulah Park Middle School is a member of the Ohio High School Athletic Association. The OHSAA is a non-profit organization whose primary purpose is to supervise and administer interscholastic athletic competition among its member schools to the end that the interscholastic program should become an integral part of the total educational program of the school. The OHSAA, through its Commissioners and the State Athletic Board of Control, develops rules and guidelines within which to operate and serve the many youths of the state of Ohio. As a member, Beulah Park Middle School agrees to abide by and cooperate with all rules and regulations of the OHSAA.
